Baptist Health wants to help you get ready for the childbirth experience by offering you the forms and information you'll need to pre-register for your admission to Baptist, South Miami or Homestead Hospital. Where you choose to deliver your baby depends on where your doctor has privileges, your insurance plan, and of course, your preferences.
The following forms and information are provided for you to fill in at your convenience. We encourage you to submit them to the maternity pre-admission office at your hospital of choice as soon as possible during your pregnancy. That way, all of the paperwork is done and everything is ready for you, whenever you need us. Take the time out to read the information provided at right -- “About Childbirth Services.”
