Under the government-mandated Health Insurance Portability and Accountability Act (HIPAA), only patients and individuals authorized by the patient may get personal and protected patient information. If you wish to get authorization, please have the patient call our Patient Financial Services representatives at 786-596-6507.

Or you may have the patient send us a fax at 786-594-6300 with their name, birth date, account number, expiration date of the authorization, statement granting you permission to accept their records for that specific account, and their signature.
