If you have already scheduled an appointment with our Scheduling Call Center, you may reschedule or cancel it by filling out the information below. Just click on the "Submit" button when you are finished, and your change/cancellation will go to a scheduling assistant. You will receive a response via e-mail or telephone within 48 hours (except on weekends and on holidays). If you prefer to change your appointment by phone, please call 786-573-6000.
