Interested in getting a bone density test for osteoporosis or a mammogram? An appointment request form is available online. Please note that a physician's prescription is required prior to making your appointment. Once you have filled out the form, your request will go to a scheduling assistant. You will receive a response via e-mail or telephone within 48 hours (except on weekends and on holidays).
