Health Education and Promotion Program: Congregational Health partnering congregations organize congregational health fairs and educational programs with the collaboration of community health resources according to the health profile of their population promoting education and medical screenings to many people without insurance.

Care Teams Program: Congregational Health partnering congregations provide practical and spiritual support to patients who are at home following a hospital stay. Each congregation has participated in the post-hospitalization spiritual and practical care course and has organized its care teams program according to the ability and mission of its congregation. Once participants implement their care teams program, they are part of the Care Teams Referral Guide and eligible to receive referrals from our hospitals’ chaplains, social workers or home care workers.

Bereavement Program: A network of Congregational Health bereavement support groups in faith settings throughout our community provides support to people who are grieving. Each congregation has participated in the Bereavement facilitator support groups training in which participants transform their losses into a meaningful gain, and obtain skills on how to integrate spiritual and grief themes into a bereavement support group. Once participants implement bereavement support groups they will be part of the Congregational Health Bereavement Resource Guide and eligible to receive referrals from our hospitals’ chaplains, social workers, home care workers, funeral homes and others.

Faith and Health Support Groups Program: Congregational Health partnering congregations provide an effective method of helping support groups of individuals with similar health challenges through health education, follow-up, and practical activities that encourage healthy choices and social support in a spiritual environment. Each congregation has participated in the faith-based health support groups training in which participants learn a model for integrating social, emotional, practical and spiritual support. Once participants implement their support groups they are part of the Faith-based Support Groups Referral Guide and eligible to receive referrals from our hospitals' chaplains, social workers, home care workers and others.
