- 2. Do I need to follow any instructions prior to my test?
- This depends on the test you're having done. Contact the scheduling department for further details.

- 3. Why do I need a prescription for my test?
- Every patient is required by Baptist Health to have a prescription for all services. Having a prescription will aid those involved in performing the exam as to what your doctor is ordering.

- 4. How long is your prescription valid?
- 6 months.

- 6. Where can I park when I arrive and what is the cost?
- There is free patient parking at Baptist Outpatient Center, South Miami Hospital, Baptist Hospital, Westchester, Palmetto Bay, Beacon and Macy's, The Falls. Valet parking is available at SMH for $3 and at BOC for $2. Baptist Medical Plaza at Coral Gables has metered parking, or garage parking at the Gables Grand Garage.

- 7. How early do I need to arrive to my appointment?
- Thirty minutes prior if your appointment is at Baptist Outpatient Center, Baptist Hospital, or South Miami Hospital. Fifteen minutes prior if you're schedules at Coral Gables, Westchester, Palmetto Bay, Beacon, or Macy's, The Falls.

- 8. What documents are required at the time of my appointment?
- You need to bring the order, or prescription, from your referring physician and your insurance card. It may be necessary for you to bring a referral or authorization number, if your insurance requires either, prior to your test. You may also be asked for a photo identification.

- 9. Doe my insurance policy outpatient diagnostic procedures?
- Your insurance carrier would be the best reference to answer this question. Contact the member services department at the number listed on your insurance card.

- 10. What do you I have to pay at the time of my test?
- You would need to contact your insurance company to find out the details of any payments that may be required. Our insurance verification department will also call to verify your benefits prior to your appointment date. Should payment be necessary, you will receive a phone call regarding the amount due at time of service.

- 11. How can I pay for my test?
- You may pay by Master Card, Visa, cash, check or money order.
