What Is Home Healthcare?
Before modern medicine, caring for someone medically in the home was quite common, and hospitalization was rare. Today, with a growing trend toward reduced hospital stays, home healthcare is gaining popularity. While caregivers of the past were generally family members or friends, home healthcare today includes a professional healthcare team working toward a common goal.

Home healthcare may be considered for patients who are:

• Recovering from an illness
• Undergoing treatment
• Disabled
• Chronically ill
• Terminally ill

With so many home healthcare options available, finding the right ones for your special needs can be confusing.  The staff at Baptist Health Home Care understands, and is eager to help.  Federal law requires that patients be informed of their options for home care, and it’s important that you be the one to decide who comes in to your home to take care of you or your loved one. Choosing the right agency can make a tremendous difference in your recovery.  Our services can bring you physical, emotional and financial benefits by allowing you to leave the hospital earlier and return to your family and the comfort of home.  We're here to do what's best for you and to assist you in locating the personal and community resources that will support you and your family.  

High Quality, High Standards
Our home health agency operates under federal and state supervision and regulation.  All employees are fully trained and qualified to provide healthcare outside the hospital setting.  They meet all federal and state requirements necessary to practice their profession.  In addition, Baptist Health Home Care is fully accredited by the Joint Commission on Accreditation of Healthcare Organizations (Florida license #HHA 21076096). Our team is highly skilled and compassionate, always delivering the quality care you’ve come to expect from Baptist Health.

In May 2003, the Centers for Medicare/Medicaid Services (CMS) began publishing 11 home health benchmarks on a quarterly basis, comparing the agency to the Florida state and national averages.  Baptist Health Home Care ranked above the national average in eight out of 10 CMS quality indicators.  Our rate of readmission to the hospital is almost half of the national average.  We focus on preventing any further complications that might cause patients to go back into the hospital.  All Baptist Health Home Care CMS quality indicators can be viewed and compared to local, state and national standards at www.cms.gov, under the Home Care Compare link.

Based on your physician’s order, Baptist Health Home Care provides the following services:

• Skilled Nursing
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Social Work Services

• Tele-home Care

Tele-home Care
Baptist Health Home Care is pleased to offer an interactive Tele-home care program for patients.  It's a new way patients can communicate with their Home Care team. 

What is Interactive Tele-home Care?
Interactive Tele-home care lets us monitor patients electronically in their homes between home visits by our staff.  The system features live video and audio, as well as medical peripherals including stethoscope, blood pressure meter, pulse oximeter, glucometer, weight scale and digital images.

The ability to "see" patients using this new technology helps us maintain closer contact and reinforce directions for their self-care.  A Tele-home care visit supplements regularly scheduled health home visits.

How it works:
A patient station, about the size of a computer, is placed in the patient's home, while the central station is based in our office.  Ordinary telephone lines link the two.  The nurse conducts a video visit by calling the patient.  The patient and nurse can see and hear each other.  The nurse gathers information, including vital signs, and takes snapshots.

How Tele-home Care Benefits Patients:

• Offers early detection and intervention, giving patients feelings of security knowing that medical attention is closer to home.
• Reports clear, concise and accurate data in a timely manner.
• Increases independence.
• Reduces emergency room visits and hospitalization.
• Enhances quality of life.

Which Patients Qualify?
Home health is appropriate for high-risk, high-cost, chronically ill patients, including those with congestive heart failure, chronic obstructive pulmonary disease, diabetes and asthma.  When criteria are met, the service is covered for Medicare patients.

Baptist Health Home Care will provide you with the system and monitoring service at no charge for a determined amount of time.  Patients need only to be willing to learn to use the system (which is very easy – think of a telephone with a television connected to it).  Nearly all patients are able to learn to use the system quickly and with little or no difficulty.

Skilled Nursing
Education and assessment of disease processes including:

Physical therapy, occupational therapy or speech therapy following illness or injuries such as:

• Fractures
• General Surgery
• Joint Replacement
• Parkinson’s Disease
• Stroke

Medical Claims and Billing Made Easy
Baptist Health Home Care services are covered by the Medicare program.  The number of visits is determined by the nature of your illness or disability. Medicare does not require that a patient be hospitalized prior to receiving home health services, provided that the patient is homebound.  A physician’s order is required to initiate services. 

Other Insurance
We're happy to provide home health services to patients who have other forms of insurance as well.  Authorization from your insurance company must be obtained before treatment begins.  We accept private insurance and worker's compensation, or we can bill you personally for services rendered.
