Employee Health Services recognizes that employees are our greatest asset in accomplishing our mission and vision. Our goal is to maintain healthy productive employees by creating a safe work environment as well as protecting them against health hazards.
Employee Health Services provides a free post-offer health screening. To expedite the process, please print the following PDF. Fill out the forms and bring them to your post-offer physical appointment. Locations vary among entities so specific instructions will be given to the employees at the time of the offer. If you are not able to print the PDF, the forms will be provided to you before your examination, or at your local Human Resources office.
