To schedule an outpatient diagnostic test, please call:

  • 786-596-2877 or 5620 for appointments at Baptist Cardiac & Vascular Institute
  • 305-434-1588 for appointments at Mariners Hospital

    Online Appointments:

    Thank you for choosing Baptist Health South Florida. To request an appointment online, please fill out the information here, or on the "Schedule Appointments" link below.  Then cllick on the "Send Appointment Request" button when you are done. 
    You can also reschedule or cancel an existing appointment.

    Your request will be sent to one of our scheduling agents who will contact you via e-mail or telephone within 48 hours (except on weekends and holidays). A physician's prescription is required for your appointment. If you prefer to make an appointment by phone, please call 786-573-6000 and one of our agents will be happy to assist you.
